The spermatocele (sperm cysts) usually occurs at the upper pole of the testis near the epididymis, as a cystic mass in the scrotum. A massive spermatocele may be difficult to differentiate from a hydrocele, which is also cystic, nontender, and transillumination. Ultrasound can help in diagnosis. Surgical removal is indicated in cases in which increases in volume and is uncomfortable for the patient.

The diagnosis is run by analyzing the contents of the cyst, which is a liquid light-colored, sticky because of the presence of sperm that are evident immediately under a microscope.

The surgical treatment involves removing the cyst, which is necessary especially when it is exposed to complications (eg. Rupture, infection, pus) or when its dimensions become such as to cause problems to the patient.
